Waking up with a headache is frustrating. Waking up with the same headache for multiple days in a row is alarming. A persistent headache for days is not something to dismiss as stress or screen fatigue, especially when it does not respond to standard painkillers. It may be your body's way of warning you about a deeper issue.

Headaches that last for more than two or three days fall under the category of prolonged or subacute headaches. Depending on their characteristics, they can point to a wide range of conditions, from manageable tension-type headaches to serious neurological emergencies.

Common Causes of Multi-Day Headaches

Tension-type headaches are the most prevalent. They produce a dull, aching pain that wraps around the head like a tight band. Stress, poor posture, and long hours in front of screens are common triggers. These can persist for days if the root cause is not addressed.

Migraine is a neurological condition characterised by moderate to severe throbbing pain, usually on one side of the head. It is often accompanied by nausea, light sensitivity, and sound sensitivity. Some migraines last 72 hours or longer without treatment.

Medication overuse headache (MOH), also called rebound headache, occurs when pain relief medications are used too frequently. Ironically, overusing paracetamol or ibuprofen can perpetuate and worsen headache cycles.

Sinusitis causes facial pressure and headache that worsens when bending forward. It is often accompanied by nasal congestion, post-nasal drip, and fever.

Hypertensive headache results from dangerously elevated blood pressure. It typically presents as a throbbing pain at the back of the head, particularly in the morning.

Intracranial pressure changes, though less common, can cause persistent headaches and may indicate serious conditions such as hydrocephalus, idiopathic intracranial hypertension, or brain tumours.

Warning Signs of a Serious Headache

Seek immediate medical attention if your headache is accompanied by:

  • Sudden onset described as "the worst headache of your life"

  • Stiff neck, high fever, and sensitivity to light (signs of meningitis)

  • Weakness, numbness, or vision changes

  • Headache following a head injury

  • Confusion or difficulty speaking

Diagnosis and Treatment

A GP will take a thorough headache history, including frequency, location, intensity, duration, and associated symptoms. Blood pressure measurement, neurological examination, and imaging such as CT or MRI may be ordered if red flag symptoms are present.

Treatment depends entirely on the cause. Tension headaches respond to stress management, physiotherapy, and non-prescription analgesics used judiciously. Migraines may require triptans, preventive medications, or Botox injections in resistant cases.

When to Visit a GP

If your persistent headache lasts more than three days, recurs frequently, or disrupts daily functioning, it is time to see a doctor. Do not rely solely on over-the-counter painkillers, as this can mask the underlying condition and lead to medication overuse headache.

At Complete Healthcare International (CHI), the international medical clinic in Singapore, doctors are skilled in evaluating both common and complex headache disorders. CHI's Orchard-based clinic provides same-day consultations, blood pressure assessments, and specialist referrals when needed.

Lifestyle Measures That Help

Alongside medical treatment, these strategies can reduce headache frequency:

  • Maintain regular sleep and wake times

  • Stay hydrated, as dehydration is a key headache trigger

  • Limit caffeine and alcohol intake

  • Take regular breaks from screens using the 20-20-20 rule

  • Practise relaxation techniques such as progressive muscle relaxation or mindfulness

Persistent headaches are common but never normal when they keep returning. Your health deserves more than temporary relief.
